In order to properly diagnose and treat patients, the staff at AAI will need information regarding your medical history, family history, current and past symptoms/medical issues and medical insurance. Patients will be asked to fill out a series of forms to provide this information.
For your convenience, forms are provided on this page, below, in PDF format.
PDF documents are easily downloaded to any computer (PC or MAC) and can be filled in by using a computer, or they can be printed out and filled in by hand.
Please bring these completed forms to your first appointment.
First time patients without completed forms:
Please arrive 30 minutes before your scheduled appointment.
First time patients with completed forms:
Please arrive 10 minutes before your scheduled appointment.

Patients with questions regarding forms should call the AAI office or simply bring the forms to their first appointment and arrive a few minutes earlier than suggested above.

Advanced Audiology Institute wants to be the standard of excellence in Las Vegas hearing and balance treatment.
